It is always our goal at Fishman & Sheridan eyeCare Specialists to give you the eye care you deserve in a timely manner. In order to ensure that your first visit goes smoothly, you may want to complete the forms below prior to your visit. You can even fax us your forms or drop them off prior to your appointment to speed up your first visit.
When you scheduled your appointment, if you provided us an email address, you should have received an email from noreply@mypatientvisit.com inviting you to register for the patient portal. You can complete your paperwork via the portal after you register. If you did not receive the invitation, or the link has expired, please call our office at (352) 360-2301 or email us at info@fishmansheridan.com, and we can send you a new invitation.
If you are unable to fill out the paperwork ahead of time, please arrive 15 minutes prior to your scheduled appointment time to complete paperwork in the office.
