A $25.00 deposit will be required to schedule an appointment online. The deposit will be applied to any copay or out-of-pocket costs for your appointment. If you cancel with less than 24 hours’ notice or do not attend your scheduled time, this deposit will be retained as a cancellation or no-show fee.
